The Brookfield Education Foundation funded the Live Well Disease Self Management Program and workshop at the Brookfield Senior Center.

Live Well is an evidence- based approach educating those with chronic diseases. Prior to initiation of this program, there were no Live Well program leaders in the Brookfield catchment area and two are required to run the program. Through this grant one person has been trained as an educator for the program. Although the program takes place at the Brookfield senior center, the program is available to patrons of all ages.

It is the expectation that Livewell will improve the participants’ ability to manage their chronic disease.
